Welcome to the Eureka Program Application Form.
Please allow for at least 30 minutes to complete this application.
We strongly recommend that you complete your application on a computer.
Please note: Due to the challenging nature of the Eureka Program, it is important that this application reflects the academic interests and abilities of the applicant only. Therefore, applicants should complete all responses in the application on their own. Any misrepresentation on your application is grounds to revoke your offer of admission into Eureka Programs.
In accordance with our Privacy Policy, the applicant must be 13 years old or older when completing this application form by themselves. If the applicant is not at least 13 years old, this application form must be completed by the applicant with a parent or guardian or completed by the applicant's parent or guardian.

Self-Introduction Videos*
Eureka Program requires all applicants to submit a 1-3 minute self-introduction video with their application. Your self-introduction video will be reviewed by the Eureka Admissions Team and it will be shared with the instructor we pair you with. Your self-introduction video should be in English.
Your self-introduction video should be 1-3 minutes long and should be in English.
In your self-introduction video, you can include or answer the following questions:

What are you passionate about and why?

Why do you want to conduct research?

What are your goals for your future?

What hobbies do you have? Are you involved in any extracurricular activities (Clubs, Sports, Music, etc.)?

How are you involved in your community? Do you volunteer or participate in any community service activities?

What awards, honors, and recognitions have you received?

Have you participated in any summer programs or camps? Please share your experience.

Writing / Project Samples*
Please attach a writing sample or a project sample with your application. All samples should be in English.
Samples can include an academic paper, research paper, portfolio, a video you created, or anything that you think best demonstrates your creativity and academic writing abilities. Writing samples should be in English and 4-10 pages long.
You may submit additional samples or samples of large sizes can be emailed to the admissions team at info@eurekaprogram.com
